Free printable Crohn's and colitis food list (fill it in from your own log)

A blank food list to fill in from your own experience: each food, how it was prepared, how much, how you felt over the next 24 hours, and your verdict. Your list, not a generic one.

US Letter, prints fine on A4. No sign-up, no email.

How to use it

  1. Print the two sheets (or download the PDF). Keep them with your food log.
  2. Add a row each time you try a food you're unsure about. Write how it was prepared (raw, cooked, peeled, blended) and roughly how much, because both can change how a food sits.
  3. Note any symptoms over the next 24 hours, then circle a verdict: fine, test again, or avoid for now.
  4. "Avoid for now" isn't forever. Many people retest foods later, especially after a flare settles. Talk to your IBD team or dietitian before cutting out whole food groups.
  5. Bring the list to appointments. A short list of foods you've actually tested is more useful than a long list of foods you've read about.
